Which diet is best for weight loss?

Meta-analysis keeps producing the same answer, and it is the answer nobody wants: at twelve months, the differences between named diets are small.

What the comparative meta-analyses show

The most useful analysis compared dietary macronutrient patterns across 14 popular named diets [4]. At six months, low-carbohydrate approaches showed a small advantage. By twelve months, the differences between diets had narrowed to the point of clinical irrelevance, and all of them had lost ground against their six-month results.

A meta-analysis of low-fat against low-carbohydrate reached compatible conclusions [5], as did a broader comparison of weight-loss effects among adults with overweight or obesity [1].

Named diets converge at 12 months

Why they converge

Every one of these diets works by producing an energy deficit, and each does it by restricting something. Low-carbohydrate removes a food category; low-fat removes a different one; the Mediterranean pattern shifts composition and displaces processed food. The mechanism differs; the arithmetic does not.

The convergence over time reflects adherence decay, which is remarkably similar across diets. People do not stop because their diet stopped working. They stop because sustaining any restriction is hard.

The Mediterranean pattern deserves a note: it is unremarkable for weight loss and has the best evidence of any dietary pattern for cardiovascular outcomes. Those are different questions, and conflating them is common.

Ketogenic diets specifically

A meta-analysis found benefits for weight loss, glycaemic control and lipid profile [3]. Two caveats matter. Early loss is inflated by glycogen and its associated water — several kilograms that return on refeeding without any fat having been regained. And adherence at twelve months is poor in most trials, which is why the long-term results converge.

Choosing, practically

  1. Pick the one you can imagine doing in a year. This is not a platitude — it is what the twelve-month data says.
  2. Prefer patterns to protocols. Something you can eat with your family beats something requiring separate meals.
  3. Protect protein and fibre regardless of the pattern. Both help with satiety and lean mass. See protein.
  4. Judge at three months, not three weeks. Early differences between diets are largely water.
  5. If you have type 2 diabetes, the calculus changes. Diet comparison in that population has been reviewed separately [2].

Common questions

Is a calorie a calorie?
For energy balance, close enough. For satiety, food-cost and adherence, absolutely not — which is why diet composition still matters even though the twelve-month totals converge.
Why did I lose 3 kg in the first week on keto?
Glycogen depletion and the water bound to it. It returns when carbohydrate does, and it was never fat.
Is the Mediterranean diet best?
For cardiovascular outcomes, it has the strongest evidence of any pattern. For weight loss specifically, it performs like the others.
Should I count calories?
Self-monitoring predicts success — see behaviour change — but the monitoring can be of intake, of weight, or of adherence to a rule. Counting is one option, not the only one.
